ERH launches new chronic disease monitoring program

ECHUCA Regional Health officially launched its Remote Patient Monitoring program on
Wednesday, December 9.


Murray Primary Health Network is funding the program across its catchment area.

“We’re working with ERH on enabling care monitoring capability for people with chronic disease, so they can be managed at home with connections to the expertise in the health service,” MPHN chief executive Mat Jones said.

“It’s really responsive and if there’s any issues, both the individual and health practitioner know.

“The really important part of it too is it increases education and helps people while they’ve got chronic disease.

“Having these resources and support, accompanied with greater understanding of their conditions and how to monitor that, keeps people healthier in the community and not requiring the services of a hospital.”

The project has the capacity for over 200 patients across regional Victoria, with 30 spots in Echuca.

Patients are given iHealth devices which track relevant symptoms for their condition and are monitored once a day by ERH’s Complex Care clinicians.

Craig Simmonds brought the iHealth devices to Australia seven years ago.

“The iHealth devices link the vital signs to their phone,” he said.

“Rather than the patient having to come into the hospital all the time, the team can monitor them at home which is so much easier for the patient and the hospital.”

The devices are being used all around Australia and took of during COVID-19 which kept most people home-bound.

The program is free for eligible people across the Campaspe region admited to the Complex Care program.
